ABSTRACT

Objective:To verify the accuracy of the Kyoto classification score of gastritis for the endoscopic prediction of the Helicobacter pylori( H. pylori)infection in Chinese population. Methods:From June 2020 to January 2021, at the Digestive Endoscopy Center of the First Affiliated Hospital of Zhejiang Chinese Medical University, the clinical data of 489 patients who underwent gastroscopy examination were collected and the gastric mucosal manifestations under white light endoscopy (including atrophy, intestinal metaplasia, widening of gastric fold, nodularity, diffuse redness, white sticky mucus, etc.) were recorded according to the Kyoto classification of gastritis. H. pylori infection of the patients was determined according to 13C-urea breath test, histopathological examination and anti- H. pylori antibody test. The Kyoto classification score of gastritis of each patient was calculated. The sensitivity, specificity and odds ratio( OR)(95% confidence interval(95% CI)) of various endoscopic appearances in prediction of H. pylori infection were analyzed. Chi-square test was used for statistical analysis. Results:Among the 489 patients, 246 patients had H. pylori infection and 243 patients did not have H. pylori infection. There were 242 patients with Kyoto gastritis score ≥ 2, among them 215 cases had H. pylori infection and 27 cases did not have H. pylori infection. The accuracy of Kyoto classification score of gastritis in predition of H. pylori infection was 88.14% (431/489). Among the five indexes of the Kyoto classification score of gastritis, there was no significant difference in the incidence of intestinal metaplasia between patients with H. pylori infection and patients without H. pylori infection ( P>0.05). The incidence rates of atrophy, widening of gastric fold, nodularity and diffuse redness were higher in patients with H. pylori infection than those in patients without H. pylori infection (68.3%, 140/205 vs. 31.7%, 65/205; 95.2%, 99/104 vs.4.8%, 5/104; 89.7%, 35/39, vs.10.3%, 4/39; 85.0%, 227/267 vs.15.0%, 40/267), and the differences were statistically significant ( χ2=45.68, 106.46, 26.37 and 283.48, all P<0.01). The sensitivity, specificity and OR (95% CI) of atrophy, widening of gastric fold, nodularity and diffuse redness in prediction of H. pylori infection were 56.91%, 73.25%, and 3.62 (2.47 to 5.29); 40.24%, 97.94% and 32.06 (12.76 to 80.57); 14.23%, 98.35%, 9.91 (3.47 to 28.35); and 92.28%, 83.54% and 60.63 (34.02 to 108.08), respectively. The sensitivity and specificity of atrophy in prediction of H. pylori infection were low, and the diagnostic efficacy was general. The incidence rates of white sticky mucus and spotty redness of the gastric fundus and body were higher in patients with H. pylori infection than those in patients without H. pylori infection (86.5%, 32/37 vs. 13.5%, 5/37; 86.9%, 146/168, vs. 13.1%, 22/168), and the differences were statistically significant ( χ2=20.96 and 137.12, both P<0.01). The sensitivity, specificity, and OR (95% CI) of the two indicators in prediction of H. pylori infection were 13.01%, 97.94% and 7.12 (2.72 to 18.60), and 59.35%, 90.95%and 14.67(8.84 to 24.34), respectively, and the diagnostic efficacy was ideal. Conclusions:The Kyoto classification score of gastritis used for the endoscopic prediction of H. pylori infection is suitable for Chinese population. However, from the perspective of individual indicator, the diagnostic efficiency of atrophy and intestinal metaplasia is unsatisfactory. At the same time, attention should be paid to whether there is sticky mucus or spotty redness of the gastric fundus and body.

ABSTRACT

Objective:To explore the association of peripheral blood neutrophil lymphocyte ratio(NLR) with primary osteoporosis (POP) in Chinese elderly women. Methods:From December 2015 to April 2019, a total of 788 elderly women in Chongqing Three Gorges Central Hospital were included in this study. They were divided into three groups according to bone mineral density: 242 cases of osteopenia, 206 cases of normal bone mineral density and 340 cases of osteoporosis.Their height, weight, statuses of bone of left femoral neck and results of blood tests were recorded, and the values of BMI, NLR and OSTA were calculated. The SPSS statistics software was used to analyze the differences of parameters among the three groups.Logistic regression analysis was used to get significant independent related factors for osteoporosis. The relationship of NLR and OSTA with T-score of left femoral neck was confirmed by Pearson correlation test.ROC curves were used to define the values of NLR and OSTA in diagnosis of osteoporosis. Results:There were statistically significant differences in age(median values: 66, 68 and 70, χ 2=71.596, P<0.001), BMI(median values: 23.46, 24.04 and 25.415, χ 2=64.936, P<0.001) and NLR(median values: 1.97, 2.435 and 2.84, χ 2=106.218, P<0.001) among the three groups(all P<0.01). Logistic regression analysis demonstrated that age, BMI and NLR were all independent significant related factors. There was correlation of NLR, OSTA with T-score of left femoral neck.In diagnosis of osteoporosis, the cut-off value, maximum Youden index, sensitivity and specificity were 2.395, 0.299, 70.3%, 59.6% for NLR, and -1.315, 0.174, 73.6%, 43.5% for OSTA. Conclusion:NLR has intermediate sensitivity and specificity in screening POP in Chinese elderly women, and deserves future researches.

ABSTRACT

Objective:To establish the fingerprint analysis method for the root of Rosa laevigata Michx from different regions by UPLC. Methods:The column was ACQUITY UPLC? Phenyl(2.1 mm × 100 mm,1.7 μm). The mobile phase consisted of methanol-water with gradient elution. The flow rate was 0. 2 ml·min-1 , the detection wavelength was 210 nm, the column temperature was 30℃, and the injection volume was 3 μl. Results:The fingerprint consisted of 15 common peaks. The range of similarity for twelve bat-ches of the root of R. laevigata Michx was 0. 489-0. 942. And the reference fingerprint of the root of R. laevigata Michx was estab-lished by UPLC. Conclusion:The fingerprint method is simple and reproducible, which can provide basis for the quality control and the medicinal resources exploration.

ABSTRACT

Objective:To study the rule of spontaneous behavior and to explore the effect on emotion of mice peripherally infected with influenza A WSN33 virus( H1N1).Methods: Mice were intranasal inoculated with H1N1 WSN33 or saline. Then mice bodyweight change, and total distance movement, average movement speed distance in the central area and feces in the open field test in 5 minutes were recorded in two weeks. Results: Mice following WSN33 infection bodyweight declined sharply until day 7 post-inoculation,and mice bodyweight recovered from influenza infection at day 8 post-inoculation.Total distance movement of mice following H1N1 WSN33 infection decreased in the open field test, and difference of the reduction was significant from day 5 to day 10 post-inocu-lation.The average movement speed had no statistical difference. The range of numbers of fecal grains was large, and they were no significant difference. Conclusion:The total distance movement decreased, but average movement speed did not change following mice infected with H1N1 WSN33.They told us that mice infected with H1N1 WSN33 had anxiety, depressed and nervous emotion which is more evident in acute stage and early recovery stage, whereas the change of the nervous emotion was small and not obvious.
